Her approach is personalized! By having direct access to our personal Doctor, we save time, effort and in total we all have improved health and wellness. It is game changing to be able to text my doctor with a question, vs sitting on the phone trying to schedule an appointment. Her ability to connect us quickly with top-notch specialists around the state has been invaluable during this time of need. What I appreciate most is the speed at which lab results come in and out and the ability to schedule an appointment quickly and efficiently without ever having to wait in a crowded and stuffy doctor's office. We've never had accessibility like this before with a doctor and her space in Leadville is so comfortable and welcoming. Rick Bieterman Dr. Lisa has helped me in so many ways over the last couple of years. • Is DPC For You? | RMFP Leadville

Is DPC Right For You? Why Direct Patient Care? Have you ever wished that you had a personal physician who could help you coordinate your care and make sure things didn't slip through the cracks? Do you worry that you will not be able to stay in your home as you get older because you just don't have the medical support you need? Have you ever gone to the emergency room and gotten stuck with a big bill for a simple treatment that you know could have easily been done in the office or over the phone? Have you been turned away by your primary care provider because their schedule is full for the day? Are you frustrated that you can’t access your primary care provider when you need them? Then DPC is right for you! If you have health insurance: It is probable that with health insurance you pay several thousands of dollars out of your pocket every year for co-pays and deductibles. With RMFP of Leadville’s Direct Patient Care practice, you pay about $100 per month or $1,200 per year. If you get most of your care in a primary care provider's office, your potential savings is thousands of dollars per year. You may be able to reduce the cost of your current insurance plan once you have primary care available anytime and as often as you need it with Dr. Lisa Zwerdlinger. If you don’t have health insurance: Many people are in the healthcare gap where they make too much money to qualify for Medicaid and too little to afford health insurance. Direct Patient Care breaks down the barrier to primary care for the uninsured. If you can pay on average $100 per month or $1200 per year, you can have excellent primary care at an affordable price. RMFP of Leadville also offers family pricing. Learn more about DPC on DPCFrontier.com Testimonials Here's what patients across the United States have to say about DPC. “As a patient, I really appreciate how my Direct Care practice handles billing. I know exactly what I’m being charged for, and how much. Not only that, but if I have any questions on my bill I can email, text, or call my doc and get a near immediate response. That’s incredible when you consider all the hours, I’ve spent on hold with billing departments over the years!” “My doctor told me about the car insurance analogy – he said medical insurance should be treated like car insurance; pay directly for things like checkups, a broken arm, and the like, and save insurance for the big-ticket items. Just like you pay for gas and oil changes directly and save insurance for accidents and major repairs! Makes perfect sense.” “I knew I’d enjoy not having to deal with insurance, but I never imagined it would be this nice. Making payments directly to my Direct Care clinic is hassle-free. I pay only for what I need when I need it. I have more control over my family’s medical expenses, and I know my family’s in good hands. It doesn’t get much better than this.” “If medical billing is this easy on me and my family, I hope it’s this easy on the docs, too! I get an email each month telling me what my invoice total is. Then I get another email to confirm my payment has gone through. I can care for my family without giving the hassle of making a payment a second thought.” ¡La mejor atención comienza aquí! Empezar

  • The Team | RMFP Leadville

A legacy of caring. Dr. Lisa Zwerdlinger “I have been practicing medicine for 27 years. I believe that health care is for the patient. It should be delivered where the patient needs it at the time that they need it, and at a price they can afford. In our current healthcare environment, insurance companies and health systems get in the way of the doctor-patient relationship. With Rocky Mountain Family Practice of Leadville, you will have access to the health care provider you have come to know and trust when you need me and not just when it is convenient for the system. It has been my pleasure and my privilege to care for you and your family for decades. I look forward to continuing that journey together.” More on Dr. Lisa ... Laurie Strasburger, PA-C laurie@rmfpleadville.com “I believe that being a good listener and giving patients all available information and empowering them to participate in their own care has the greatest impact.” Laurie Strasburger has been practicing medicine as a Physician Assistant for 27 years. A native of Denver, Laurie received her Bachelor of Arts degree from Colorado College in Colorado Springs, then graduated from the University of Colorado Health Sciences Child Health Associate Program in 1996. She has been providing care to patients mostly in rural areas in Colorado, Alaska, and California since. Laurie is at home in Leadville and has cared for this community previously in both St. Vincent Health’s ER and Rocky Mountain Family Practice with Dr. Zwerdlinger. Laurie is passionate about delivering quality medical care to patients without the constraints of time, insurance, or financial barriers. She is well-versed in chronic disease, urgent and emergency care and she is fluent in Spanish. When she is not in the office, Laurie can be found in nature with her partner, Doug, and canine companion, Yuki, skiing, biking, running and laughing. She also plays drums, paints (by numbers) and volunteers as a mentor in Lake County. Claudia Vargas, MA Practice Manager claudia@rmfpleadville.com Claudia Vargas serves many essential roles at Rocky Mountain Family Practice. She is Dr. Lisa’s right hand. Her passion for patient care and removing barriers for non-English proficient patients is her guiding force. As a young girl Claudia and her family suffered a tragedy when her mother passed away due to the language barrier between her mother and her caregivers. The tragedy has resulted in Claudia’s inspiration for choosing her career in healthcare. Claudia loves that at RMFP of Leadville, patient care is not rushed; she can get to know her patients and identify how to help them take care of themselves through their treatment plan and by connecting them to resources as needed. Claudia graduated as a Medical Assistant in Sept 2009 from Intellic College in Grand Junction, CO. She has worked in healthcare since 2011. She moved to Leadville in 2015 and fell in love with the healthcare providers and the community in Lake County. Claudia has 1 daughter and 1 son and 2 stepchildren with her husband. The family enjoys cooking and hiking together. A favorite summer pastime is picking strawberries at Turquoise Lake.

  • Frequently asked questions | RMFP Leadville

Frequently asked questions and answers What is Rocky Mountain Family Practice of Leadville? Rocky Mountain Family Practice of Leadville (RMFP) is a primary care practice that offers Direct Primary Care (DPC). This healthcare model was intentionally chosen by Dr. Lisa Zwerdlinger because it focuses on patient care, clinical excellence, accessibility, and service. By adopting the DPC model RMFP has put patient care first. What is DPC? Direct Primary Care (DPC) is an innovative alternative payment model for improving access to high-functioning healthcare with a simple, flat, affordable membership fee. No fee-for-service payments. No third-party (insurance) billing. Patients have extraordinary access to a physician of their choice and physicians are accountable first and foremost to their patients. (Direct Primary Care Coalition, 2021). Learn more about DPC at DPCFrontier.com Additional benefits of DPC and RMFP include: • Direct contact with your doctor when you need them • More time with your doctor • Simple pricing schedule • No Co-pays or insurance billing issues • After-hours access to providers by phone, text, or email • Affordable healthcare options to assist business owners in offering healthcare options to employees. How do appointments and scheduling work with RMFP? Rocky Mountain Family Practice of Leadville is membership-based. We ensure that patients can see Dr. Lisa when they need her care by limiting the number of members. Members just reach out via phone, email, or text to have access to Dr. Lisa Zwerdlinger. RMFP is about delivering healthcare where the patient needs it when they need it. A member of RMFP will be seen when they request care and as often as they need/want to be seen. Appointment lengths are dictated by what the patient needs to address in that visit as opposed to how many patients can be squeezed into the day. Dr. Lisa Zwerdlinger will see patients in person at the office, on telehealth, or coming to where the patient is in Lake County. Does Rocky Mountain Family Practice of Leadville take insurance? At RMFP, everything we do is with the patient in mind. We keep costs down and provide timely access to medical care by offering an inclusive fixed-monthly fee. By not billing insurance for the care we provide in the office, we minimize the medical practice’s administrative workload while increasing the time we spend with patients. You are still able to use your insurance if services are needed outside of our office like specialty consultations with other doctors, tests at outside facilities, and services provided in hospitals for example. Most patients save money overall by not needing to pay copays, meet deductibles, or have unnecessary testing or specialist/urgent care visits. As your physician will be focused on you, your care is well coordinated and you always have access to your physician. Do I need to keep insurance? Yes, though with an RMFP membership, you may desire to adjust your coverage plan to focus more on prescriptions, and major medical expenses such as emergency, imaging, labs, or hospitalization to decrease your overall costs. If you have questions about your Medicare supplement insurance or Medigap and how choosing a less expensive option could help you save money, please visit Supplement Insurance (Medigap) plans in Colorado (medicare.gov) or State Health Insurance Assistance Program.
